8-4:30 pm. every Monday, Tuesday, Thursday and e.o Wednesday. Pay Range $30.70 - $46.05Major Responsibilities:
Provides ongoing Medical Nutrition Therapy (MNT) care for oncology patients through screening, prioritizing, assessing and recommending interventions. Adapts workflow to accommodate fluctuations in acuity, specific patient needs. Develops and implements a plan of care for patients based on an assessment of nutritional needs. Uses physical, clinical, and laboratory data to complete assessment including calculations of nutrition requirements to make sound recommendations for optimal nutrition. Provides follow up on nutrition care plan to facilitate continuity of care. Communicates with physicians, nurse practitioners, physician assistants, nurses, medical staff, therapies within oncology and outside medical departments regarding nutritional care of the patient. Documents results of nutrition assessment, medical nutrition therapy recommended and initiated, plan of care results and recommendations for necessary changes in diet to provide optimal nutritional intake. Maintains accurate recordkeeping. Prescribes enteral nutrition support by assessing the patient and calculating macronutrients and fluid needs. Evaluates patient tolerance and effectiveness of formula and manages initiation or modifications to regimes. Demonstrates extensive knowledge of specialize nutrition support and triages issues and concerns in collaboration with providers. Coordinates and initiates home care for enteral support. Assists team in entering nursing home care orders as needed. Provides education to patients and families on feeding tube site cares, feeding initiation and fluid flushing needs and use. Involves clinic staff in process as needed. Coordinates parenteral nutrition with physicians and home care pharmacists. Calculates macronutrients, micronutrients, fluid and electrolyte needs in collaboration with infusion pharmacists. Assists in managing and triaging concerns associated with parenteral nutrition. Provides nutrition education through individual and group teaching to patients and families, staff and community based on needs assessment. Counsels on proper nutrition including food selection, preparation and meal planning based on lifestyle, oncology and other medical needs. Develops nutrition education materials as needed. Provides MNT by prescribing therapeutic diets, oral nutrition supplements, vitamin/mineral use. Provides education and counseling on the use of prescribed or recommended over-the- counter dietary supplements for safety, to minimize interactions with medications and treatments. Assists in dosing pancreatic enzymes to improve and correct exocrine pancreatic insufficiency. May prescribe vitamins and supplements in partnership with providers using fullscript application. Provides survivorship nutrition to all eligible patients, participating in multi-disciplinary clinic style care as able. Participate in multi-disciplinary care conferences for site specific disease as requested and required. Participate in accreditation processes as requested. Participate in cancer committee functions as required by the American College of Surgeon's Commission on Cancer. Assist in closing care gap for nutrition care for patients in various care areas including medical oncology, radiation oncology, surgical oncology, survivorship. Leads efforts to identify, treat and prevent malnutrition. Performs Nutrition Focused Physical Assessment of patients to evaluate the loss of specific muscles and subcutaneous body fat to determine the presence of and degree of malnutrition. Documents appropriately to support inpatient needs for malnutrition reimbursement. Provides MNT education to patients identified to have moderate or severe malnutrition to improve patient outcomes and positively impact hospital readmission rates. Provides day-to-day training; Plans learning experiences, teaches and evaluates performance of interns/students. Reviews, approves, and co-signs the work of dietetic interns. Mentors interns/students to provide well-rounded clinical and community rotation. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age specific information is developed further in the departmental job standards. Licensure, Registration, and/orCertification Required:
Dietitian certification issued by the Commission of Dietetic Registration (CDR) Dietitian (RD) registration with the Academy of Nutrition and Dietetics, and Dietitian certification issued by the state in which the team member practicesEducation Required:
Minimum of Bachelor's Degree if CDR registration eligibility was obtained prior to January 1, 2024 or minimum of Graduate (master's, practice doctorate or doctoral degree) if initial CDR registration was obtained on or after January 1, 2024 Successful completion of Accreditation Council for Education in Nutrition and Dietetics (ACEND) accredited supervised practice programExperience Required:
Typically requires 2 years of experience in a dietitian role, inpatient setting preferred.Knowledge, Skills & Abilities Required:

Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders and twist frequently during the work shift.
