Job Description

Job Summary The Outpatient Dietitian performs a variety of specialized duties related to the nutrition counseling of patients / families for Medical Nutrition Therapy (MNT) diet education. This may include but are not limited to diabetes education, weight management, cardiovascular disease, renal disease, malnutrition / weight gain, infants with failure to thrive, cancer and nutrition therapy, gastrointestinal disorders and nutrition therapy, sports nutrition or healthy diet. The Certified Diabetes Care & Educat ion Specialist assists with the coordination and delivery of Diabetes Self-Management Education (DSME) services. Utilizing age specific criteria, ensures all services are provided in accordance with appropriate policies , procedures, ADA Recognition guidelines, and federal and state regulation . Responsibilities Core Duties/Job-Specific Obtains the patient's weight, height (as needed), blood pressure and heart rate. This is documented in the Electronic Health Record. Interviews patients / families which may include obtaining a diet history, exercise habits, reviewing emotional/social issues as part of the diet counseling process and documents any pertinent information obtained in the Electronic Health Record. Makes appropriate referrals to other services as indicated. Counsels patients / families on medical nutrition / diet therapy principles and restrictions. Utilizes available medical nutrition therapy education materials or obtains them from reputable sources. Documents the nutrition counseling session in the Electronic Health Record on the appropriate templates. Registers patients for Diabetes Self-Management Classes Obtains patient medical and insurance information from referring providers. Contacts providers to obtain referral for interested patients. Schedules or reschedules outpatient nutrition counseling appointments as needed and maintains patient files. Develops or researches appropriate internet sites or evidence-based materials as needed and time permits, for additional nutrition educational materials. Revises the Outpatient Dietitian Electronic Health Record templates as needed. Assists in projects to increase patient referrals as needed Work with inpatient hospital staff on nutrition and diabetes related issues as needed. Participates in departmental and hospital wide mandatory in-services and meets the mandatory annual in-service requirements in utilities management, infection control, safety and blood-borne pathogens. Participates in community health care events as time permits. Other duties as assigned Competencies and Skills Actively participates as a member of the management team; shares information appropriately, returns telephone calls within one day of receipt, and constructively contributes to committees and teams. Establishes and maintains effective, cooperative relationships with others, including peers, supervisors, and customers. Written communication is clear, effective, and in accordance with MHS style guidelines. Responds positively and appropriately to organizational and departmental change Knowledge, Skills & Abilities 1 year in diabetes education with 1 year additional experience in related healthcare field. Ability to positively affect and maintain consumer and community support for DSME services. Effective planning, analyzing, decision making, human resources management, and organizational skills. Effective interpersonal communication skills in order to manage staff and promote DSME services. Strong negotiation skills in order to acquire the necessary contracts and services.
