Job Description
We are seeking an experienced Director of Nursing (DON) to lead the clinical operations of our skilled nursing facility. We are looking for a hands-on nursing leader with strong SNF experience who knows how to keep a building survey ready every day. The ideal candidate is proactive, organized, and accountable. They understand CMS requirements, identify clinical and compliance risks before they become deficiencies, develop strong nursing teams, and ensure residents consistently receive high-quality care. Responsibilities Maintain continuous survey readiness and compliance with federal, state, and facility requirements. Conduct routine rounds, clinical audits, documentation reviews, and staff competency checks to identify and correct potential deficiencies. Lead clinical preparation and response for annual surveys, complaint surveys, and other regulatory visits. Oversee nursing operations and ensure residents receive safe, appropriate, high-quality care. Monitor key clinical areas including falls, wounds, infections, weight loss, changes in condition, medications, hospitalizations, and documentation. Ensure assessments, care plans, physician orders, treatments, and nursing documentation are accurate and completed timely. Lead QAPI initiatives and ensure corrective actions and Plans of Correction are implemented and sustained. Ensure incidents, complaints, abuse/neglect allegations, and reportable events are properly investigated, documented, and followed up. Lead, coach, and hold RNs, LPNs, CNAs, and nursing leadership accountable for performance and standards of care. Ensure nursing staff are properly trained, competent, and prepared to meet resident needs. Oversee nursing staffing based on census and resident acuity while managing overtime and agency utilization. Work closely with the Administrator, ADON, MDS, Infection Preventionist, physicians, pharmacy, and interdisciplinary team. Communicate effectively with residents, families, staff, and healthcare providers. Qualifications Active RN license in good standing in Colorado or ability to obtain appropriate Colorado licensure. Skilled nursing facility experience required. Previous DON, ADON, Unit Manager, or comparable SNF leadership experience strongly preferred. Strong knowledge of CMS regulations, F-tags, state surveys, Plans of Correction, QAPI, and SNF compliance. Proven ability to lead and hold nursing teams accountable. Strong clinical judgment, organization, communication, and problem-solving skills. PCC or similar long-term care EMR experience preferred.
