Job Description

Position Summary Sun River Health & Rehab is seeking an experienced and compassionate Director of Nursing (DON) to lead our nursing team and oversee all clinical operations within our skilled nursing facility. This key leadership role is responsible for ensuring the delivery of exceptional resident care while maintaining compliance with all federal, state, and local regulations. The Director of Nursing provides strategic leadership, operational oversight, and clinical direction for the Nursing Department. The ideal candidate is a strong leader with experience in long-term care who is passionate about resident-centered care, staff development, regulatory compliance, and quality improvement. Working closely with the Nursing Home Administrator, Medical Director, Regional Nurse Consultant, physicians, therapists, pharmacy partners, and other healthcare providers, the Director of Nursing ensures the highest standards of care are maintained 24 hours a day. Essential Duties and ResponsibilitiesClinical Leadership Direct and oversee all nursing services within the facility. Ensure the delivery of high-quality, resident-centered care consistent with regulatory standards and best practices. Develop, implement, and maintain nursing department policies, procedures, and clinical protocols. Collaborate with the Medical Director and Regional Nurse Consultant to improve clinical outcomes and resident satisfaction. Ensure appropriate resident assessments, care planning, and ongoing monitoring are completed. Staff Leadership & Development Recruit, hire, onboard, mentor, and retain qualified nursing personnel. Supervise Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Certified Nursing Assistants (CNAs). Conduct performance evaluations and provide coaching, corrective action, and professional development opportunities. Create and manage nursing schedules to ensure adequate staffing levels and compliance with staffing requirements. Foster a positive, collaborative, and accountable workplace culture. Regulatory Compliance & Quality Assurance Ensure compliance with all federal, state, and local regulations governing skilled nursing facilities. Lead survey preparation efforts and respond professionally during state and federal inspections. Develop and oversee Quality Assurance and Performance Improvement (QAPI) initiatives. Monitor clinical outcomes and implement corrective action plans as necessary. Stay informed of regulatory changes and update facility practices accordingly. Resident Care Management Oversee resident admissions, discharges, transfers, and care coordination processes. Ensure comprehensive care plans are developed, implemented, and updated as needed. Serve as a liaison between residents, families, healthcare providers, and facility staff. Assist in evaluating prospective admissions to ensure appropriate care needs can be met. Clinical Systems Oversight Manage and lead facility clinical programs, including: Infection Prevention and Control Program Controlled Substance Management Psychotropic Medication Monitoring Skin Integrity and Wound Care Programs Weight Management and Nutrition Monitoring Risk Management Programs Hospice Services Coordination Quality Assurance Performance Improvement (QAPI) Programs MDS & Reimbursement Compliance Oversee completion, accuracy, processing, and transmission of Minimum Data Set (MDS) assessments. Ensure resident interviews, assessments, and care plans are completed in accordance with regulatory requirements. Monitor clinical documentation practices to support compliance and reimbursement integrity. Operational Management Manage nursing department budgets and monitor nursing-related expenditures. Oversee nursing supply inventory, equipment maintenance, and resource utilization. Coordinate with ancillary service providers including pharmacy, laboratory, radiology, hospice, and therapy partners. Participate in facility leadership meetings and strategic planning initiatives. Administrative Responsibilities Attend required meetings, trainings, and continuing education programs. Complete all required documentation accurately and timely. Perform additional duties as assigned by the Nursing Home Administrator or Regional Nurse Consultant. In the absence of the Nursing Home Administrator, assume responsibility for overall facility operations. QualificationsRequired Graduate of an accredited school of nursing. Current, unrestricted Registered Nurse (RN) license in the state of practice. Strong knowledge of skilled nursing facility regulations and standards. Excellent leadership, communication, organizational, and problem-solving skills. Proficiency with electronic medical records and computer systems. Ability to successfully complete pre-employment screening requirements, including a criminal background check. Preferred Minimum three (3) years of nursing experience. Prior leadership or management experience in a skilled nursing, long-term care, rehabilitation, or post-acute care setting. Experience with MDS, care planning, regulatory compliance, and survey management. Physical Requirements Ability to stand, walk, sit, bend, and move throughout the facility for extended periods. Ability to lift, transfer, or assist with loads exceeding 30 pounds when necessary. Ability to respond quickly to emergencies and resident care needs. Ability to work in a fast-paced healthcare environment while managing multiple priorities. Ability to work with residents experiencing dementia, mental health challenges, chronic illnesses, and disabilities. Schedule & Availability Full-Time Position Must be available for occasional evenings, weekends, holidays, and on-call responsibilities as needed. Pay:
$120,000.00 - $140,000.00 per year
