Job Summary We are seeking a dynamic and compassionate Wellness Director/Director of Nursing (LPN) to lead our Personal and Memory Care community with enthusiasm and expertise. This pivotal role combines clinical leadership, regulatory compliance, and team management to ensure the highest quality of resident care. The ideal candidate will bring a proactive approach to managing nursing staff, overseeing care plans, and maintaining a safe, compliant environment that promotes resident well-being and staff excellence. This position offers an exciting opportunity to make a meaningful difference in the lives of residents while fostering a positive, energetic workplace culture. Duties Lead and supervise nursing staff, including nurse supervisor and direct care providers, ensuring effective shift management and team collaboration Develop, implement, and review individualized care plans for residents, ensuring they meet medical, emotional, and social needs Oversee compliance with long-term care regulations, Medicare requirements, HIPAA standards, and state licensing laws Manage clinical documentation review, medical records accuracy, and EMR/EHR system updates to ensure regulatory adherence and quality assurance Coordinate interdisciplinary team efforts across assisted living, memory care, hospice care, home health, and acute care services to deliver comprehensive resident-centered care Conduct regular audits of medical records and documentation to uphold high standards of patient care and regulatory compliance Foster a culture of continuous improvement through staff training in medical terminology, physiology, anatomy, and best practices in nursing leadership Skills Extensive assisted living experience with a strong understanding of Personal can Memory Care communities. Proven experience with expertise in managing care teams within regulated environments Knowledge of Pennsylvania regulations. Familiarity with EMR/EHR systems for documentation management and patient records tracking Strong leadership skills with supervising experience in nursing administration and nurse management roles Ability to oversee complex care plans involving memory care, patient safety protocols, and specialized resident needs Deep understanding of long-term care regulations including Medicare guidelines and HIPAA compliance Excellent communication skills for coordinating with healthcare professionals, residents' families, regulatory bodies, and team members Competence in managing documentation review processes and ensuring accuracy in medical records management Join us as we dedicate ourselves to providing exceptional resident care through energetic leadership and innovative clinical practices.
