Community (Non‑Medical, Personal Care Environment) Role Summary The Director of Nursing (LPN/RN) provides leadership in resident wellness, personal care oversight, service plan management, and coordination of outside healthcare services in a Personal Care Home (PCH) setting. This role operates within Pennsylvania's 55 Pa. Code Chapter 2600, which establishes PCHs as non‑medical communities that do not provide skilled nursing services. The title reflects internal operational responsibility; it is not a DHS‑regulated nursing position. We are looking for supervisory experience. Key Responsibilities Oversee resident wellness, changes of condition, and coordination with external healthcare providers. Lead development and updates of resident service plans (non‑clinical, compliant with PCH standards). Oversee medication administration processes and staff training. Train and support direct care staff in personal care delivery and documentation. Assist Administrator with regulatory compliance and DHS inspections. Maintain service documentation and support incident reporting and follow‑up. Serve as a primary contact for families regarding resident wellness and care coordination. Qualifications Active LPN or RN license in Pennsylvania. Experience in personal care, assisted living, memory care, or long‑term care preferred. Strong understanding of resident assessment and service planning in a non‑medical residential setting. Effective leadership, communication, and organizational skills.
Pay:
$75,000.00 - $85,000.00 per year
