Job Description

Wheatland Health Community Care is seeking an experienced Assistant Director of Nursing / Field Registered Nurse to support the Director of Nursing with day-to-day clinical operations, staff oversight, quality assurance, and patient care. This is a working clinical leadership role combining nursing supervision with field support. Field visits are primarily for admissions, urgent clinical needs, staffing gaps, and overflow coverage rather than a fixed daily patient caseload. Key Responsibilities Support the Director of Nursing with daily clinical operations and nursing oversight. Provide skilled nursing visits as needed for admissions, resumptions of care, recertifications, urgent clinical needs, staffing shortages, and overflow coverage. Review clinical documentation for accuracy, completeness, timeliness, and compliance. Assist with chart audits, plans of care, physician orders, recertifications, missed visits, hospitalizations, and documentation follow-up. Provide clinical guidance, mentoring, orientation, competency support, and performance feedback to field clinicians. Monitor changes in patient condition and ensure significant clinical concerns are appropriately escalated and coordinated with physicians and the interdisciplinary team. Assist with case conferences, quality-improvement activities, infection prevention, and survey readiness. Coordinate with the Operations Manager regarding staffing gaps, scheduling priorities, field coverage, and workflow needs. Identify and address clinical or operational barriers affecting patient care and service delivery. Participate in clinical on-call rotation when assigned. Maintain timely completion of required clinical and supervisory documentation. Coverage Area The primary service area includes Seminole, Orange, Brevard, Osceola, and Volusia counties. Field travel is based on agency and patient needs and is primarily intended to support admissions, urgent visits, staffing shortages, and overflow coverage. This position is not intended to carry a fixed full-time field caseload. Qualifications Current, unrestricted Florida Registered Nurse license. Current CPR certification. Minimum of two years of RN experience. At least one year of home health nursing experience required. Home health leadership, supervisory, clinical coordinator, case management, or preceptor experience strongly preferred. Experience with OASIS, Medicare home health documentation, plans of care, physician orders, and recertification processes strongly preferred. Strong clinical judgment, leadership, communication, organization, and documentation skills. Ability to work independently and manage both supervisory and field responsibilities. Valid Florida driver's license, reliable insured transportation, and ability to travel throughout the assigned service area. Ability to maintain a secure and private home-office environment.
Pay:
$85,000.00 - $95,000.00 per year
