About NextCare Providers LLC NextCare Providers LLC is an Arizona-based home health agency providing skilled nursing services to individuals and families in the comfort of their homes, including individuals receiving services through Arizona's Division of Developmental Disabilities (DDD) . Our care model is built around one-on-one, patient-centered nursing care supported by a coordinated clinical and administrative team. Our organizational structure includes clinical leadership, nursing staff, scheduling and staffing support, Quality Assurance (QA), Human Resources, billing, and administrative support. We work collaboratively to maintain continuity of care, timely documentation, regulatory compliance, and effective communication between our nurses, patients, families, providers, and facilities. We believe strong clinical leadership is essential to supporting nurses in the field and maintaining the quality of care our patients deserve. About the RN ADON Role The RN Assistant Director of Nursing works directly with the Director of Nursing to support the day-to-day clinical operations of the agency. Unlike a strictly administrative nursing position, this is a hands-on role . The RN ADON will divide their responsibilities between clinical oversight, office-based administrative duties, supervisory responsibilities, and patient care in the field. The position requires someone who can confidently move between reviewing documentation in the office, supporting a nurse with a clinical concern, communicating with a provider or family, completing a supervisory visit, and helping leadership resolve an operational or patient-care issue. Key ResponsibilitiesClinical Leadership & Nursing Oversight Assist the Director of Nursing with the daily oversight and coordination of nursing services. Provide clinical guidance and support to LPNs, RNs, and other applicable nursing staff. Serve as a clinical resource for field nurses when questions or patient-care concerns arise. Identify clinical, documentation, staffing, or performance concerns and assist with appropriate follow-up and resolution. Promote adherence to physician orders, plans of care, agency policies, and applicable clinical standards. Escalate significant patient-safety, compliance, staffing, or operational concerns to the DON and appropriate leadership. Support continuity of care when staffing changes, call-outs, or other clinical concerns occur. Supervisory Visits & Patient Care Conduct required supervisory visits every two (2) weeks . Perform regular or supplemental patient visits as needed. Assess patient status and identify changes or concerns requiring follow-up. Evaluate the quality and appropriateness of care being provided in the home. Communicate with patients, families, nurses, physicians, facilities, and other members of the care team as appropriate. Follow up on clinical concerns and physician orders. Ensure services are being delivered according to the established plan of care. Admissions, Recertifications & Discharges Complete and/or assist with patient admissions . Complete required recertifications and ongoing clinical assessments. Coordinate and complete patient discharges when appropriate. Review clinical information and documentation necessary to support continuity of care. Communicate changes in patient status and care needs to the appropriate members of the interdisciplinary team. Ensure required clinical documentation is completed accurately and within established timeframes. Quality Assurance & Compliance Support the agency's Quality Assurance (QA) and clinical compliance processes. Review nursing documentation for accuracy, completeness, consistency, and timely submission. Identify documentation deficiencies and follow up with nursing staff regarding necessary corrections. Monitor compliance with applicable agency policies and clinical requirements. Assist with internal and external audits and corrective follow-up. Assist with identifying trends or recurring concerns that may require additional training or process improvement. Help ensure clinical operations remain aligned with applicable home health, DDD, AHCCCS, and agency requirements. Incident Management & Clinical Follow-Up Assist with the review and investigation of patient-care incidents and clinical concerns. Prepare and/or review Incident Reports (IRs) as required. Participate in clinical fact-finding and follow-up. Gather objective information from nurses, patients, families, facilities, and other involved parties when necessary. Assist with determining appropriate clinical follow-up and corrective measures. Ensure significant clinical concerns are communicated through the appropriate chain of command. Nurse Training & Development Assist with nursing orientation and clinical onboarding. Support competency verification and patient-specific training. Participate in the development and administration of nursing in-service education . Identify opportunities for additional nurse education based on documentation reviews, incidents, clinical concerns, or regulatory requirements. Provide constructive clinical guidance and follow-up to nursing staff. Promote accountability, professional communication, and adherence to agency standards. On-Call Responsibilities The RN ADON will participate in the agency's clinical on-call rotation with the Director of Nursing and designated nursing staff . During assigned on-call periods, responsibilities may include responding to clinical questions from field nurses, assisting with urgent patient-care concerns, addressing unexpected staffing or coverage issues, communicating with providers or families when appropriate, and determining when additional clinical or emergency intervention is necessary. The RN ADON must be dependable and responsive during assigned on-call periods and appropriately escalate significant concerns to the DON or other leadership when necessary. Communication & Technology Strong communication and technology skills are essential for this position. The RN ADON will communicate regularly with field nurses, the DON, administrative leadership, QA, scheduling, patients, families, PCPs, facilities, and other members of the care team .

Candidates should be highly comfortable navigating electronic systems, reviewing clinical documentation, communicating electronically, and learning agency-specific workflows. Qualifications Required Current and unrestricted Registered Nurse (RN) license in the State of Arizona Current CPR/BLS certification Valid Arizona Fingerprint Clearance Card Strong clinical assessment and nursing judgment Strong understanding of clinical documentation and compliance requirements Excellent written and verbal communication skills Strong organizational, prioritization, and time-management skills Ability to effectively support and communicate with nursing staff Ability to work independently and collaboratively as part of a leadership team Proficiency with Microsoft Teams and electronic communication/documentation systems Reliable transportation Valid driver's license Ability to travel to patient homes and report to the Company office as required Ability to participate in the required on-call rotation Preferred Previous home health nursing experience WellSky/Kinnser experience Previous supervisory, management, charge nurse, or nursing leadership experience
