Set the Clinical Standard. Make an Impact. Nayar Health Care is seeking an experienced, hands-on Director of Nursing (DON) to lead our Richmond Home Health clinical operation. This is not an entry-level leadership position. We are looking for a proven home health nursing leader who understands how to manage people, maintain clinical quality, drive accountability, and keep an agency survey-ready every day—not just when a survey is approaching. Candidates should have approximately 2 years or more of Director of Nursing experience in home health. Candidates without comparable home health leadership experience are unlikely to be considered. What You'll Do As Director of Nursing, you will provide day-to-day clinical leadership and oversight for the Richmond Home Health team, including: Lead and manage clinical staff while establishing clear expectations and accountability Oversee the quality and coordination of patient care Ensure Plans of Care accurately reflect the patient's needs, goals, homebound status, medical necessity, and ordered services Provide oversight of OASIS, SOC, ROC, recertification, discharge, and other clinical documentation Monitor documentation timeliness, accuracy, and completion Review clinical records and identify concerns before they become compliance issues Ensure physician orders and Plans of Care are complete and followed Monitor hospitalization rates, utilization, recertifications, discharges, and other clinical outcomes Work closely with scheduling, intake, therapy, sales, QA, and executive leadership Maintain compliance with Medicare Conditions of Participation, CMS requirements, state regulations, and accreditation standards Participate in QAPI and performance improvement initiatives Assist with hiring, orientation, coaching, competency, and performance management of clinical staff Address clinical concerns promptly and professionally Help ensure patients are receiving the right care, at the right frequency, for the right clinical reason Maintain ongoing survey readiness
What We Are Looking For Required:
Current unrestricted Virginia RN license Approximately 2+ years of Director of Nursing experience in home health Strong working knowledge of Medicare-certified home health Knowledge of OASIS, Plans of Care, homebound status, medical necessity, and home health documentation requirements Demonstrated experience leading and holding clinical teams accountable Strong organizational, communication, and problem-solving skills Ability to manage multiple priorities in a fast-paced clinical environment
Highly Preferred:
Experience with ACHC accreditation Experience managing a larger or growing home health census Previous responsibility for clinical quality, survey preparation, QAPI, and regulatory compliance Strong understanding of home health utilization and clinical outcomes The Leader We Want We are looking for someone who does more than hold the title of DON.
