Comprehensive benefits package included About the Role Stonebridge Nursing and Rehab is seeking a dynamic, leadership-driven Speech-Language Pathologist / Director of Rehab to head our therapy department. This is a dual-purpose role designed for a skilled clinician who thrives on managing a dedicated team while maintaining a passion for direct patient care. In addition to leading the therapy program at Stonebridge, you will also play a key role in collaborating and assisting with treatments at our partner facility, Fairview Rehabilitation & Healthcare .

Key Responsibilities Leadership & Administration:

Oversee the daily operations of the rehabilitation department, including scheduling, clinical programming, and ensuring compliance with state and federal regulations.

Clinical Excellence:

Provide high-quality speech-language pathology evaluations, treatment plans, and therapy to residents experiencing communicative, cognitive, or swallowing disorders.

Cross-Facility Support:

Assist with patient treatments and clinical collaboration at Fairview Rehabilitation & Healthcare as needed.

Team Collaboration:

Partner with nursing leadership, physicians, and families to integrate rehab goals into comprehensive, interdisciplinary care plans.

Documentation:

Maintain accurate, timely, and compliant electronic medical documentation. Qualifications Master's Degree in Speech-Language Pathology from an accredited program. Current, unrestricted state licensure as an SLP. Certificate of Clinical Competence (CCC-SLP) required. Prior experience in a leadership or Director of Rehab role within a skilled nursing (SNF) or long-term care setting is highly preferred. Strong communication, organizational, and interpersonal skills.
