Job Description

Director of Rehabilitation Services / Staff Physical Therapist Highland Nursing Home, a 140-bed Skilled Nursing Facility in Massena, NY, is looking for a Staff Physical Therapist to lead our growing Rehab team. We employ a supportive environment, staff-friendly strategies, and a team-oriented approach to make sure our staff are well-trained, well-looked after, and superbly appreciated. We are conveniently located right off Route 37 in Massena, NY, 10 min away from the beautiful Robert Moses State Park and the town of Cornwall, Canada.
PRIMARY FUNCTIONS
: Oversees the clinical management of the rehab department including overseeing of case management, quality improvement, care planning, clinical utilization, and patient identification Evaluates and creates plans of care for patients with variety of diagnosis Develops an appropriate plan of care for Physical Therapy Assistants to complete. Manages scheduling for all staff Provides quality therapy services (will have own clinical caseload) in accordance with established standards of practice, therapy department procedures and productivity standards Recruits and retain all therapy service personnel; providing effective orientation, on-the-job training, and performance reviews Comprehension of multiple reimbursement systems including: Medicare Part A, Medicaid CMI, Medicare Part B, HMO management Reviews resident specific RUG category per assessment period. Coordinates change in MDS data according to community procedure Experience with PPS and MDS management preferred Attends morning reports, weekly and monthly quality assurance meetings Closely collaborates with the Administrator, regional consultants and local area referral sources to maintain working relationships and to ensure continued referral sources and excellent customer satisfaction Results oriented with strong critical thinking and problem solving skills
QUALIFICATIONS
