Job Description

Director of Rehabilitation (DOR) Creekside Care Center of Gahanna is seeking an experienced and compassionate Director of Rehabilitation to lead our therapy team. This is an excellent opportunity for a licensed PT, OT, PTA, COTA or SLP who is ready to combine clinical expertise with team leadership. You will oversee rehabilitation services, mentor therapists, support strong clinical outcomes, and help ensure each resident receives personalized, high-quality care. Our in-house rehabilitation team works collaboratively to create individualized care plans that help residents regain function, improve independence, and achieve their highest possible quality of life. This role provides leadership across all therapy disciplines and uses clinical judgment, performance data, and industry best practices to ensure services are appropriate, effective, and focused on achieving each resident's discharge goals. The DOR will also maintain compliance with federal, state, and local regulations, promote a safe treatment environment, and build strong relationships with residents, families, staff, and facility leadership. Key Responsibilities Lead and mentor the rehabilitation team Manage staffing, scheduling, and productivity Ensure accurate, timely, and compliant documentation Support clinical programs and positive resident outcomes Participate in hiring, training, and performance management Monitor department performance, reports, and budget goals Maintain strong relationships with staff, residents, families, and facility leaders Qualifications Current PT, OT, PTA, COTA or SLP with the state, in good standing Rehabilitation leadership experience preferred Skilled nursing or long-term care experience preferred Strong clinical, communication, and organizational skills Knowledge of Net Health Electronic Documentation System preferred Make a meaningful impact on residents, families, and fellow clinicians while advancing your career as a rehabilitation leader.
