Job Description
Liberty Healthcare seeks to contract with Licensed Clinical Social Workers and Licensed Marriage and Family Therapists who will work on an as needed basis to conduct client evaluations for the California Conditional Release Program. About the program The California Conditional Release Program is a statewide program which exclusively serves adult sex offenders who are transitioning from the Coalinga State Hospital into the community. With community safety as the top priority, the program seeks to support clients so they may live healthy and productive lives in the community. Program objectives Community safety Preventing client recidivism Transitioning clients from hospital treatment into the community Securing housing and community supports Proactively addressing client needs and risk factors Providing behavioral health services, case management, 24/7 monitoring, and transportation for clients Work setting and schedule You will work per diem (as needed and as available) Report writing may be done remotely from your home office in California In-person evaluations will be done onsite at the Coalinga State Hospital in Coalinga, CA To achieve greater efficiency and minimize travel, several evaluations are scheduled for each visit to Coalinga. Role description As a consultant with Liberty Healthcare, your work will involve evaluating patient's progress in treatment and identifying factors that may preclude patients from conditional release. Responsibilities Traveling to the Coalinga State Hospital for onsite in-person client meetings Evaluating patients through face-to-face meetings Reviewing patient files and meeting with treatment staff Writing summary reports Invoicing for travel expenses and services provided Compensation $500.00 for each completed evaluation Reimbursement for approved travel expenses to Coalinga, CA
