Job Description
Job Overview We are seeking a compassionate and clinically skilled Mental Health Therapist to provide psychotherapy services to geriatric patients in skilled nursing facilities, assisted living communities, memory care units, rehabilitation centers, and other long-term care settings. The Mental Health Therapist will work as part of an interdisciplinary behavioral health team and collaborate closely with psychiatric providers, nursing staff, social services, facility leadership, residents, and families. This Mental Health Therapist position provides an opportunity to make a meaningful impact on the emotional well-being, behavioral health, and overall quality of life of older adults. The ideal candidate is compassionate, dependable, clinically confident, and committed to providing high-quality behavioral health services while maintaining timely and accurate documentation. Duties Conduct comprehensive behavioral health and psychosocial assessments. Develop individualized treatment plans based on patient needs, clinical findings, and treatment goals. Provide individual psychotherapy to geriatric patients. Provide family and caregiver support and education. Diagnose and treat behavioral health conditions within the scope of licensure. Monitor patient progress and modify treatment plans as clinically indicated. Provide behavioral interventions for residents experiencing challenging behaviors. Provide crisis intervention and de-escalation when necessary. Collaborate with psychiatric nurse practitioners, supervising physicians, nursing staff, social workers, and other members of the care team. Participate in interdisciplinary care plan meetings as requested. Communicate effectively with facility staff and families regarding patient needs and treatment. Maintain accurate, timely, and compliant clinical documentation. Complete all required documentation within established timeframes using the electronic health record (EHR). Maintain established productivity standards. Comply with
HIPAA, CMS
regulations, state licensing requirements, and company policies. Travel between assigned facilities as needed. Experience Minimum of one year of clinical therapy experience. Experience working with geriatric or long-term care populations. Skilled Nursing Facility (SNF) experience. Assisted Living or Memory Care experience. Knowledge of Medicare and long-term care regulations. Experience working alongside psychiatric providers. Crisis intervention and de-escalation experience. Knowledge of dementia care and behavioral management techniques. Required Qualifications Master's degree in Social Work, Counseling, Marriage & Family Therapy, Psychology, or a related behavioral health field. LCSW- Licensed Clinical Social Worker LPC
