Nexus Family Healing is hiring a residential Mental Health Therapist to add to our clinical team in Manteno, IL! Provide overall management of youth's treatment through active engagement, clinical assessment, treatment, and discharge planning. Nexus Family Healing is a national nonprofit with over 50 years of experience restoring hope for children and families through mental health services, foster care, adoption, and residential treatment. At Nexus Family Healing in Manteno, IL, youth in residential care also attend Safe Harbor School, an accredited special education program for grades 6-12, offering individualized curricula with transferable credits upon discharge. Schedule/Pay:

Full-time; on-site : 101 N Bramble Street, Manteno, IL 60950 Salary range: $68,000

• $75,000 annually based on years of related experience and credentials.

Position Summary :

Provide overall management of youth's treatment through active engagement, clinical assessment, treatment, and discharge planning. Therapist will provide clinical insight and work from a trauma informed lens to enhance the milieu and programming. Engage with families through family therapy and treatment planning. Collaborate with the community and providers related to the youth's progress and discharge planning.

Responsibilities:

Creates and drives a trauma-informed environment Collaboration with youth and family to work with a team and develop a Nexus Individual and Family Guidance Plan Engage in and have active involvement of the Family Collaborative Intake meetings Complete Child and Adolescent Needs and Strengths (CANS) functional assessment in accordance with the TCOM (collaborative) principals Provide brief, individualized, youth-focused, strength and evidenced-based family and individual therapy models with youth and families. Participate in all youth related treatment team meetings Assess readiness for discharge in conjunction with the treatment team Coordinate/communicate aftercare needs of the client with family and all necessary social service and community providers to ensure a smooth transition Communicate with the treatment team to include all desired and necessary family members, educational, legal, community, and social service providers (as well as all-natural supports that the youth and family identify) to gather all necessary information to inform and support the treatment process Ensures that all required client reports, records, and all documentation are produced accurately and maintained in a consistent, timely, and professional mann Equal employment opportunity, including veterans and individuals with disabilities.
