Job Description

Job Summary Greenleaf Consulting Group LLC is seeking a compassionate, licensed, and dedicated Home-Based Therapist to join our growing behavioral health team serving children, adolescents, and families throughout Allen County. This is a field-based, community-based, home-based therapy position providing therapeutic services in clients' homes, schools, community settings, courtrooms, and the office as needed. Therapists work directly with children and families to improve emotional well-being, strengthen family functioning, promote resilience, and help clients achieve individualized treatment goals through trauma-informed, evidence-based behavioral health services. As a Therapist, you will complete behavioral health assessments, develop individualized treatment plans, provide individual and family therapy, coordinate care with community providers, and collaborate with the Indiana Department of Child Services (DCS) and other community partners to support positive outcomes for children and families. Duties Provide home-based individual and family therapy in homes, schools, community settings, and office locations. Complete comprehensive behavioral health assessments and diagnostic evaluations. Develop, implement, and update individualized treatment plans based on client needs and progress. Utilize evidence-based and trauma-informed therapeutic interventions. Provide crisis intervention and safety planning when clinically indicated. Assist clients in developing healthy coping skills, emotional regulation, communication skills, and problem-solving strategies. Provide individual and family therapy while collaborating with caregivers to implement therapeutic strategies that reinforce treatment goals, strengthen family functioning, and promote positive behavioral and emotional outcomes. Attend Child and Family Team Meetings, court hearings, school meetings, staffing meetings, and other required appointments. Collaborate with DCS Family Case Managers, schools, physicians, placement providers, probation officers, attorneys, and other community agencies. Monitor treatment progress and modify interventions based on clinical needs. Maintain timely, accurate, and professional clinical documentation in accordance with agency policies, state regulations, Medicaid requirements, accreditation standards, and DCS service standards. Maintain confidentiality while adhering to ethical and legal standards of clinical practice. Manage an assigned home-based, community-based caseload while meeting productivity and documentation expectations. Participate in clinical supervision, staff meetings, trainings, and ongoing professional development. Qualifications Master's degree in Social Work, Counseling, Marriage and Family Therapy, Psychology, or another qualifying behavioral health field from an accredited institution. Active Indiana clinical license or license eligibility preferred (LCSW, LMHC, LMFT, HSPP, or Associate License). Candidates working toward independent licensure are encouraged to apply if supervision requirements can be met. Experience providing therapy to children, adolescents, and families preferred. Experience in home-based therapy, community mental health, child welfare, family preservation, outpatient therapy, or behavioral health services preferred. Knowledge of trauma-informed care, child development, attachment, family systems, and evidence-based treatment models. Knowledge of or ability to learn and implement the Indiana Department of Child Services Practice Model. Strong clinical assessment, treatment planning, communication, documentation, and problem-solving skills. Ability to work independently while managing a home-based, community-based caseload . Proficiency with Microsoft Office and electronic documentation systems. Requirements Master's degree in a qualifying behavioral health field. Active Indiana clinical license or license eligibility preferred. Valid driver's license with a safe driving record. Reliable personal vehicle and current automobile insurance. Ability and willingness to travel throughout Allen County and surrounding counties. Ability to provide home-based therapy services in homes, schools, community settings, courtrooms, and office locations. Ability to work flexible hours, including occasional evenings, based on client and family needs.
