Job Description

Hegira Health, Inc. (HHI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HHI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages.
Two Positions:
Adults SMI - position will focus on the completion of annual paperwork and post-hospitalization appointments for commercial/Medicare clients Adults SUD A hybrid work schedule (three days in office and two days remote is available after completion of training/90 days).
REQUIREMENTS
A Master's Degree from an accredited college or university, specializing in psychology, social work, counseling, or other related human services field Current LMSW issued by the State of Michigan One year providing direct treatment services to adults in a behavioral health inpatient or outpatient setting Demonstrate knowledge of diagnostics, psychopharmacology, and supportive treatment approaches as applied to a severely mentally ill (SMI) adult population Knowledge of the identification and treatment of co-occurring mental health and substance abuse disorders
RESPONSIBILITIES
(summary): Facilitate new patient orientation group, as assigned, and provide accurate information regarding clinic and corporate CMH services. Complete assigned new intake comprehensive bio-psychosocial assessment within seven days of interview, as necessary. Formulate preliminary DSM five-axis diagnosis. Identify issues that may exist regarding safety (personal and environmental), natural supports, and community involvement. Identify referrals needed for comprehensive treatment, including medical, nutrition, substance abuse, STD/HIV, education, and vocational training. Identify Case Management needs and complete referral recommendation, as indicated, within seven days of identified need. Obtain releases of information for coordination of care, including primary care physician, past treatment episodes, emergency and significant support persons, court, and school related contacts. Educate patient and patient's parent/guardian with respect to admission information packet, including but not limited to, Medicaid Grievance procedures and Recipient Rights processes. Complete Urine Drug Screen, as indicated. Schedule appointment for psychiatric evaluation within appropriate timeframe relative to medication supplies and/or acuity of symptoms. Complete an individualized Treatment Plan for each patient by the fourth session or within 30 days. Utilize the patient's Person Centered Questionnaire in development of treatment plan and support person participation. Complete Treatment Plan Reviews (TPR) minimally after the first 90 days of treatment and 180 days thereafter. • Update Treatment Plan via a TPR following a change in clinical status. Document on Treatment Plan criteria and anticipated date for planned discharge. Complete Discharge paperwork within 21 days of discharge date. • Complete a Continuing Care Plan for each patient discharge at the last session for planned discharges and within seven days for unplanned discharges. Report medication issues, including non-compliance and adverse side effects, to clinic psychiatrist and/or clinic supervisor immediately. Establish effective therapeutic bonds with patients assigned to caseload. Pay is based upon years of related experience .
