Responsibilities:
Provides individuals, family, and group counseling to persons with diverse diagnosis for senior/disabled adults. Development of plans of care Implementation. Work as part of a multidisciplinary team to ensure high quality outcomes for patients. Patient assessments and creative engaging activities to enhance group therapy. Assist patients in obtaining services to enhance their health and wellbeing.
Qualifications:
Masters degree in Counseling/Social Work. 1-3 years experience in Clinical Therapy Licensure required LCSW or LPCC We're all about well-being, starting with yours.
