Primary Purpose:
This position is a split role that involves being a contracted member of the interdisciplinary team at Rogue Valley Mentoring (RVM) supporting the health, safety, and impact of mentoring relationships within RVM's 1:1 Match Program as well as carrying a small caseload of outpatient clients through Kairos Jackson Services' outpatient program. At RVM, this role will hold the title of RVM Clinical Support Coordinator and report to the Match Program Director. At Jackson Services, this role will hold the title of Individual & Family Therapist and report to their assigned site supervisor. In both roles, this position is responsible for building trusting relationships while maintaining clear ethical and professional boundaries, applying a strength-based, culturally responsive approach, and maintaining flexibility and adaptability in collaborative team-based settings.
Education and Experience:
Must meet Qualified Mental Health Professional (QMHP) classifications, which includes a degree in one of the following: Graduate degree in Psychology; Graduate degree in Counseling; Graduate degree in Social Work; Graduate degree in Recreational, Art, or Music Therapy; or Graduate degree in a behavioral science field. Must be licensed in a clinical field (LCSW/LPC/LMFT, etc.) and have two years' post-licensure experience in direct mental health service delivery. Experience in case management, behavioral health, and community-based services required. Additionally, bilingual (English/Spanish) and bicultural lived experience reflective of the community served is preferred. Must demonstrate the ability to conduct an assessment, including identifying precipitating events, gathering histories of mental and physical health, substance abuse, past mental health services and criminal justice contacts, assessing family, cultural, social and work relationships, conducting a mental status examination, complete a DSM diagnosis, write and supervise the implementation of a Service Plan, and provide individual, family or group therapy within the scope of their training.
Minimum Qualifications:
Provide proof of current, valid Oregon Driver's License (or if out-of-state will obtain Oregon license within 30 days), comprehensive automobile insurance (if applicable) and a safe driving record (minimum of 3 yrs.) to use company or personal vehicle for required duties. Successfully pass a Criminal Background Check and Medicaid Fraud Check. Obtain and maintain current certification in the Child and Adolescent Needs and Strengths (CANS) assessment. Obtain and maintain current Crisis Prevention Intervention (CPI) certification and demonstrate proficiency in Certification procedures. Obtain and maintain current First Aid and Cardiopulmonary Resuscitation (CPR) certification. Obtain and maintain National Provider Identifier (NPI) number and credentialing application process. Essential Duties, Responsibilities and Core Competencie Deliver and coordinate psychiatric services for outpatient youth, young adults, and families in a collaborative team-based environment. Demonstrate knowledge of all applicable Oregon Administrative Rules (OAR) as they pertain to assessment, treatment planning, and ongoing documentation of services for outpatient settings. Manage a caseload of RVM mentor-youth matches with regular, proactive check-ins. Support match sustainability and long-term relationship development. Maintain accurate, timely, and thorough documentation across settings. Bring a licensed clinical perspective to case management, ethical dilemmas, and program decisions. Support mentors in responding appropriately to complex youth needs. Participate in youth/family intake and onboarding processes and determine eligibility using program eligibility matrix. Support youth voice, choice, and agency Provide resource navigation and referrals for families. Ensure services are culturally responsive, affirming, and accessible. Develop and facilitate training, onboarding support, and feedback processes in partnership with Match Program Director. Provide administrative support for program events. Collaborate with referring partners (schools, ODHS, healthcare providers, nonprofits). Participate in continuous quality improvement and assurance activities. Demonstrate knowledge of youth and young adults' development and group dynamics by adjusting therapeutic approaches to developmental level and group interactions. Safely de-escalate youth and young adults, including in crisis situations, by effectively using verbal intervention skills.
