We seek to be a valued outpatient mental health treatment center in the community that is trusted with the care and uplifting of our clients throughout their journey of positive change. Job description HCLFF provides wrap-around services in the areas of Outpatient Mental Health Clinic (OMHC), Psychotropic Medication Management, and Psychiatric Rehabilitation Program (PRP). We are seeking to hire a Part Time Therapist who has experience working in the social service field and providing therapy services. Essential Responsibilities Complete mental status and psychosocial assessment of clients Provide individual and family therapy services, through telemedicine, implementing goal specific and measurable treatment plans. Refer client's to the Psychiatric Rehabilitation Program (PRP) when appropriate Complete and review individual treatment plans with clients twice a year Consult regularly with psychiatrists on member care Prepare and submit reports on mental health activities as requested Participate in staff development meetings and in quality of care activities on a monthly basis Manage panel and practice management within departmental guidelines. Performs other related duties as assigned Qualifications Ability to work with clients ages 5-18 and adults Patient coordination experience: minimum 1 year (Required) Behavioral Health experience: minimum 2 years (Required) Microsoft Office (Word, Excel, PowerPoint, etc.) proficiency: minimum 1 year (Required) Previous experience with Electronic Health Record (EHR) preferred
