Job Description
Hegira Health, Inc. (HHI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HHI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages.
AFTERNOON SHIFT AVAILABLE
Monday through Friday 12:00noon - 8:30pmMENTAL HEALTH THERAPIST REQUIREMENTS
Master's Degree from an accredited college or university, specializing in psychology, social work, or guidance and counseling Obtain and maintain licensure/certification issued by the State of Michigan (LARA) Provided direct treatment services to adults in a behavioral health inpatient or outpatient setting for 18 months or more not less than 18 months, of which at least 6 months' time was spent working with adults with co-occurring disorders Basic knowledge of diagnostics, psychopharmacology, and supportive treatment approaches as applied to a severely mentally ill (SMI) adult population Knowledge of the identification and treatment of co-occurring mental health and substance abuse disorders Possess and maintain a valid Michigan chauffeur's license or obtain during the hire processMENTAL HEALTH THERAPIST RESPONSIBILITIES
(summary): Intake Complete assigned new intake comprehensive bio-psychosocial assessment within seven days. Formulate preliminary DSM five-axis diagnosis. Identify issues that may exist regarding safety (personal and environmental), natural supports, and community involvement. Identify referrals needed for comprehensive treatment. Obtain releases of information for coordination of care. Educate patient and patient's guardian with respect to admission information. Schedule appointment for psychiatric evaluation. Treatment Planning and Progress Reviews Complete an individualized Treatment Plan for each patient. Treatment Plan problem areas, goals, and objectives address the patient's DSM diagnosis. Demonstrate use of natural supports in Treatment Plan. Complete Treatment Plan Reviews after the first 90 days of treatment and 180 days thereafter. Update Treatment Plan following a change in clinical status. Discharge Process Document on Treatment Plan criteria and anticipated date for planned discharge. Update changed discharge criteria. Discharge patients with no contact for 30 days. Complete a Continuing Care Plan for each patient discharge. Ongoing Coordination of appointments. Provide linking and coordination of community-based services. Transport patients as needed to community-based services. Provide individual supportive, solution-focused psychotherapy not less than once every 14 days. Document all patient contacts. Initiate contact to non-compliant patients within 24 hours of a missed appointment. Communicate with hospital staff in the event of an inpatient admission, prior to the patient's discharge from the hospital. Meet with patient in hospital within two working days of notification of hospitalization. Assist patients and/or other clinical staff with various case management tasks i.e., medication compliance, referrals, linkage to resources. Respond to patients within 24 hours. Report medication issues to clinic psychiatrist and/or clinic supervisor immediately. Consult with psychiatrist and/or nurse within five days of the identification on a non-emergent medical condition. Complete required annual documentation. Maintain therapeutic relationships within boundaries established. Critical Thinking Provide services at frequency and intensity consistent with individual patient diagnosis and clinical status. Maintain a broad-based knowledge of referral sources, both internal and external. Consult with CSS in the event a patient presents at risk of harming self or others. Pay is based upon years of related experience and licensure.Job Type:
