Job Summary We are seeking a passionate and energetic PHP Clinician to join our dedicated team. The PHP Dual Diagnosis Therapist may be scheduled to treat onsite for groups held on weekdays, weekends, evenings and occasional holidays. The PHP Dual Diagnosis Therapist will focus on behavioral health, addiction, recovery and aftercare plan. The PHP Dual Diagnosis Therapist also works as a part of
WARRIORS HEART
Treatment Team members. PHP Dual Diagnosis Therapist may also conduct groups such as Assignment Group, Family Education, Life Skills/Didactic Groups, Relapse Prevention and others, with the appropriate training.

  • Able to be held accountable by the PHP Clinical Manager and maintain an open communication with Psych Nurse Practitioner regarding medications, observations or side effects, etc.
DUTIES and
RESPONSIBILITIES
  • Conduct the Intake Biopsychosocial Assessment on caseload assigned by the PHP Clinical Manager and document in the company EHR within 24 hours of intake.
  • Formulate Dual Treatment Plan within 5 days of being assigned a client based on accumulated information from pre-treatment records, client interview, assessment or discussions with other professionals, family members and/or other client's loved ones, etc.
  • Provides professional support to all
WARRIORS HEART
clients.
  • Communicating with the client's family members and loved ones (with signed release).
  • Document all clinical interactions with clients and loved ones within 48 hours of communication into the company EHR.
  • Attend weekly multidisciplinary treatment team meetings and provide clinical insight to observations of
WARRIORS HEART
clients.
  • Communicate with other professionals within
WARRIORS HEART
milieu to evaluate your client's progress.
  • Provide clinical recommendations for continued support and treatment.
  • Recommend
WARRIORS HEART
Alumni group to clients prior to discharge.
  • Develop an Aftercare plan, and gain client signature, for all of your individual clients at least five days prior to discharge.
  • Complete a Discharge Summary within 48 hours of your client's discharge.
  • Commit to continued personal development and training.
  • Report directly to PHP Clinical Manager.
