Job Description

We are focused on what matters most to patients. Their goals and preferences are the drivers of the Gadzoom model of care. Gadzoom Health is a growing medical directorship group that partners with skilled nursing facility communities to serve their residents. We are looking for a highly qualified Licensed Mental Health Counselor (LMHC), Licensed Clinical Mental Health Counselor (LCMHC), or Licensed Marriage and Family Therapist (LMFT) who can help drive our vision and support our partners' patient populations. This is a part time position with a flexible work schedule and opportunities to grow. This role may serve patients in St. George. Ideal candidates live in or near St. George or are otherwise able to commute reliably. Required Credentials (Candidates should have one of the following):
Licensed Mental Health Counselor Licensed Clinical Mental Health Counselor Licensed Marriage Family Therapist Candidate Responsibilities:
Conduct therapy initial evaluations and follow-ups Create pharmacological care plans Collaborate with social work department and customers Work on a collaborative team with primary care, psychiatrical, and medical practitioners Assist in clerical documentations for progress and outcomes E04JI802q4gp409asl7
