Role:
The Diabetes Care Director will lead and oversee comprehensive diabetes management programs aimed at improving patient outcomes across diverse healthcare settings. This role involves strategic planning, implementation, and continuous evaluation of diabetes care initiatives to ensure adherence to the latest clinical guidelines and best practices. The director will collaborate with multidisciplinary teams including physicians, nurses, dietitians, and educators to develop patient-centered care plans and educational resources. Additionally, the role requires monitoring program effectiveness through data analysis and quality improvement measures to drive enhanced care delivery. Ultimately, the Diabetes Care Director will serve as a clinical and operational leader dedicated to advancing diabetes care and supporting patients in achieving optimal health.
Minimum Qualifications:
Registered Nurse (RN), Certified Diabetes Educator (CDE), or equivalent clinical credential. Bachelor's degree in Nursing, Public Health, Healthcare Administration, or a related field. Minimum of 5 years of experience in diabetes care or chronic disease management. Demonstrated leadership experience in a healthcare setting, preferably managing multidisciplinary teams. Strong knowledge of diabetes pathophysiology, treatment modalities, and current clinical guidelines.
Preferred Qualifications:
Master's degree in Nursing, Public Health, Healthcare Administration, or a related discipline. Advanced certification such as Certified Diabetes Care and Education Specialist (CDCES). Experience with healthcare quality improvement methodologies and data analytics. Familiarity with electronic health record (EHR) systems and diabetes management software. Proven success in program development, grant writing, or community health initiatives.
Responsibilities:
Develop, implement, and manage diabetes care programs that align with organizational goals and regulatory standards. Lead and coordinate multidisciplinary teams to deliver integrated diabetes education, treatment, and support services. Monitor patient outcomes and program performance metrics to identify areas for improvement and implement evidence-based interventions. Provide clinical expertise and guidance to healthcare providers on diabetes management protocols and emerging therapies. Facilitate training and professional development opportunities for staff to enhance diabetes care competencies. Engage with patients and community stakeholders to promote awareness and access to diabetes prevention and management resources. Ensure compliance with healthcare regulations, accreditation requirements, and quality assurance standards related to diabetes care.
