JOB PURPOSE
Collaborates with clinical team members to facilitate unmet health care needs of our patients and their families. This occurs through health promotion and health education activities for individuals and groups, outreach activities in the community, coordinating group visits in the clinic settings, receiving referrals from clinical team members for addressing basic needs and advocacy, reinforcing needed education and self-management skills, coordinating care management with other clinical team members, and documenting in the EMR. The primary goal of the El Rio Health Clinic Health Advisor is to support El Rio's Mission of providing comprehensive, quality health care that is affordable and accessible to all who may have healthcare needs, by successfully performing the primary essential functions.
Essential Job Functions:
Provides direct services to potential and current patients for improving their health and self-management skills. Rounds daily with clinical team and attends clinical team meetings to identify patients in need of follow up, education and additional advocacy and/or support with self-management skills. Documents all patient services in the EMR/EHR as needed for clinical excellence. Provides accurate and high-quality screening testing as directed by the clinical team. Identifies and learns more about health concerns and illnesses reflective in specific clinic populations through education; therefore, able to provide basic health education for patients and families. (i.e., diabetes, seniors, high-risk pregnancy, immunization, etc.) Plans and prepares teaching materials and equipment necessary for prescribed curriculum. Advocates for patients and families when they are in need, with the intention of building self-management skills. Maintains data related to all programs provided, coordinated, and offered for patients and the community including but not limited to: the number of participants those needing follow up. screening tools for measure with an emphasis on outcomes Follows up with all patients regarding desired education, group visits or other health care needs within 48 hours of query or referral. Coordinates and assists with planning and logistics for community events and events at specific clinic locations. Greets patients, visitors and community at various specific clinic events and activities. Maintains exceptional level of customer service; address and resolve complaints, if possible; serve as an example by providing excellent service to internal and external customers, employees, and patients. Collaborates with clinical team and other community agencies to ensure quality care for our patients and their families. Teaches culturally appropriate, effective health promotion and disease prevention classes and workshops for target population at specific clinic locations. Support efforts to manage change and innovation and foster teambuilding. Collaborates with Clinic Manager to manage expenses related to community education and outreach through annual budget and various grant funding sources. Collaborates with patient and clinical team to maintain health to avoid costly emergency room visits and hospital care when possible. Orchestrates monthly activities and education for a specific health center location and in concert with other El Rio Health Center locations. Provides monthly outreach and presentations for community agencies, churches, schools, and other identified groups in need of access to health care. Assures that individuals and community members are aware of how to access health care services and establish a "health care home" at various health center locations. Contacts targeted participants and providers to assure class and/or group visit attendance, adherence to self-management activities, and follow up with providers. Maintains a current diverse list of community agencies and resources for patients and their families to maintain and improve their health status. Creates and updates a monthly calendar of health center programs, activities, and outreach events. Provide education and presentations to community organizations, agencies, businesses, health fairs, churches, and house parties. Conducts community health outreach and recruitment strategies to contact and enroll individuals. Strategies include, but are not limited to, telephone contacts, home visits, public health events and follow-up on health center referrals. Contributes to team effort and complies with all facility policies and procedures including but not limited to those addressing HIPAA and Compliance and performs other duties as assigned. Maintains a clean, safe, and hygienic work environment in compliance with all Policies and Procedures including but not limited to work areas, workstations, examination rooms, hand washing, infection prevention and control etc. for this position. Demonstrates an understanding of and proficiency with the application of all compliance and reporting requirements respective to Joint Commission Certification (JCC) standards.
Minimum Education and Experience:
Bachelor's Degree in Healthcare or Public Health or related field from an accredited university. One (1) year of experience working as a Community Health Advisor or advocacy role in a human services organization or agency. If applicable, equivalent combination of education and experience may be considered, and must be directly related to the functions and responsibilities of the job.
Required Licenses, Certifications, and Registrations:
Level I fingerprint clearance card: current valid and in good standing or have applied for it within seven working days after beginning employment. Employees in this position are required to have reliable transportation that can meet any operational reassignments of the organization during the workday. If an employee is driving during work hours, the employee is required to possess a valid driver's license and must comply with Arizona vehicle insurance requirements.
Preferred Education, Experience, Skills, Abilities:
Community Health Advisor (CHA) Certification Bilingual (English/Spanish) with the ability to speak, read and write in both languages. New hires and transfers must submit their fingerprint clearance card or fingerprint receipt before their hire date or transfer date. Level One (1) Non-IVP Fingerprint Clearance card must be received within 30 days after the new employee hire date or transfer date. All employees are strongly recommended to obtain and maintain vaccination status (i.e., as recommended by CDC and/or other public health agencies) to include an Influenza vaccination.
