Salary:

Position Purpose:
To assist LVIM patients diagnosed with diabetes or pre-diabetes on behavior modification and lifestyle changes to improve management of blood glucose levels.
Position Summary:
The Diabetes Nurse Educator (DNE) is a grant-funded position and has the primary responsibility to provide healthy living support to patients who are diabetic or pre-diabetic. The DNE will work closely with referring APRNs and MDs and play an integral part in creating patients' health plans as well as patients' successful achievement of health plan goals. Services may include, but are not limited to, health coaching, regular follow-up, and creating health plans that address diet and physical activity. Principal Duties and Responsibilities Participates in collaborative care model with referring APRNs and MDs. Develops care plans for pre-diabetic and diabetic patients in order to prevent disease exacerbation, improve outcomes, increase patient engagement in self-care, decrease risk status, and minimize hospital and ED utilization. Provides accountability to patients and regular monitoring of health plans. Purchases necessary supplies within budget and tracks expenses accordingly. Plans, coordinates, and executes DEEP (Diabetes Empowerment Education Program) classes, in person or virtually. Appropriately documents interactions/plans/results in the electronic health record. Understands and abides by criteria set forth in the grant funding document. Tracks data related to the Healthy Living program and reports quarterly. The Diabetes Nurse Educator will be expected to successfully complete v-Fusion Training after hiring. (Expense covered by grant funds; see attached for details.) Qualifications/Skills Excellent oral and written communication skills Exceptional customer service skills Proven ability to handle confidential information and work with minimal supervision Proficient with MS Office Suite, including Word, Excel, and Outlook Thorough knowledge of modern office equipment, including multi-phone lines systems, fax, copiers, scanners, etc. Ability to maintain cooperative working relationships with other departments and organizations Be service-oriented with the ability to pay attention to details in a fast-paced and changing environment Education & Experience Requirements RN credentials required At least two years of medical experience College degree preferred Experience with EMR (electronic medical records) and medical terminology Diabetes Education certification or background preferred.
Reporting Relationships:
Clinic Manager Typical Physical Demands The position requires some lifting and filing of documents; Some local travel may occasionally be required which will be reimbursed as applicable; Sensitive records will routinely be handled and/or observed in this position, records that fall under regulated security and confidentiality laws and regulations; Occasional after-hours events may require attendance. Common Expectations of LVIM Team Members Responds to correspondence and requests for collaboration in a timely manner; Performs quality work within deadlines with or without direct supervision; Participates in training, leading, and supervising volunteers in your department; Presents needs for new volunteer positions to the Director of Workforce Management. Discusses volunteer HR concerns or need for new volunteer position with Director of Workforce Management; Participation in LVIM's annual fundraising and volunteer events
