About This Role:
Care Coordination & Patient Engagement :
Accepts warm hand-offs from medical and behavioral health providers to ensure seamless integration of care; engages patients (adults and children) and families in setting and achieving self-care goals through individualized care planning and support.
Health Education & Coaching :
Delivers in-person and virtual health education sessions—both individual and group—using evidence-based curricula to support patients' self-management and wellness.
Case Management & Support Services :
Provides case management, tracks patient progress, coordinates referrals to community resources, and monitors ongoing care plans to ensure medical, behavioral, and psychosocial needs are met. Scheduling & Access to
Services :
Coordinates and schedules appointments across multiple service lines, including medical, dental, nutrition, and behavioral health; educates patients on available Evara Health services and facilitates access.
Reporting & Committee Participation :
Collects and reports program data, presents patient case reviews, participates in CMHI Oversight Committee, and supports special projects and organizational compliance with HIPAA, HRSA, and other regulatory standards.

Education and Experience:
Bachelor's Degree in Social Work, Health or Human Services field, preferred Minimum of three years' experience with minimum qualifications listed above
