Erie Family Health Promo/LHL Health Educator(Humboldt Park) AmeriCorps - 4.3 Chicago, IL Job Details Full-time 6 hours ago Benefits Health insurance Qualifications High school diploma or GED Full Job Description The Health Educator delivers 1:1 health education visits, facilitates group sessions, completes disease management assessments, conducts outreach, and schedules appointments. This position also supports our Lending Hands for Life program by conducting group classes for patients with HIV and supporting outreach for patients with HIV to ensure that they are following up with care. This position requires someone with deep commitment to providing compassionate care that honors and individuality of every patient.
Member Duties :
Assesses patients' educational needs and barriers Perform brief health education interventions using approved curriculum, in consultation with the care team. Conducts a minimum of 85 1:1 health education visit per month Responsible for promoting group programming including identifying appropriate patients and recruiting a minimum of 10 patients per month for groups. Facilitates a minimum of 1 and maximum of 3 group sessions per month Documents patient care, care team communication, resulting action steps.
