JOB SUMMARY
The Wellness Coordinator is responsible for conducting health screenings, assessing client needs, and coordinating enrollment into wellness programs such as weight management, diabetes prevention/management, and other chronic condition initiatives. This role serves as a key liaison between clients, healthcare providers, and program resources to ensure individuals receive personalized, effective wellness support.
QUALIFICATIONS
Bachelor's degree in Health Promotion, Public Health, Nursing, Exercise Science, or related field. Experience in wellness coaching, health screenings, or chronic disease management programs (e.g., diabetes, weight loss). Knowledge of biometric screening techniques and health risk assessments. Strong interpersonal and communication skills. Ability to handle sensitive health information with confidentiality (HIPAA compliance). Proficiency in data entry and wellness program tracking systems. Certification such as Certified Health Education Specialist (CHES), Certified Wellness Coach, or Diabetes Educator (CDCES). Experience with population health management or corporate wellness programs. CPR/AED certification. Bilingual skills are a plus.
ROLES & RESPONSIBILITIES
Coordinate comprehensive health screenings, including biometric measurements (e.g., BMI, blood pressure, glucose levels). Evaluate client health status, risk factors, and readiness for behavior change. Identify individuals eligible for programs such as weight management, diabetes prevention/education, and lifestyle coaching. Maintain accurate and confidential client health records. Coordinate program logistics, scheduling, and follow-up appointments. Collaborate with healthcare providers, nutritionists, and fitness professionals to ensure integrated care. Serve as the primary point of contact for client questions and support. Track program participation and outcomes (e.g., weight loss, A1C improvements). Analyze data to evaluate program effectiveness and identify opportunities for improvement. Prepare reports for leadership and stakeholders. Ensure all screenings and programs comply with applicable regulations and company policies. Maintain certifications and stay current with wellness and chronic disease management best practices.
Pay:
$60,000.00 - $75,000.00 per year
