Overview We are seeking a dedicated and knowledgeable Diabetes Nurse Educator specializing in Maternal and Child health to join our dynamic healthcare team. This role involves empowering patients through education, managing complex care plans, and collaborating with multidisciplinary teams to optimize health outcomes for mothers and children affected by diabetes. The ideal candidate will possess strong clinical expertise, excellent communication skills, and a passion for patient-centered care. Responsibilities Provide comprehensive diabetes education tailored specifically to maternal and pediatric populations, including preconception, pregnancy, and postpartum stages. Develop individualized care plans that incorporate nutrition management, blood glucose monitoring, and lifestyle modifications. Conduct diagnostic evaluations to assess patients' understanding of their condition and identify areas requiring additional support. Offer health coaching to promote behavioral changes that support long-term diabetes management and weight loss goals. Supervise and mentor nursing staff or student interns involved in diabetes education programs. Collaborate with physicians, dietitians, and other healthcare professionals to coordinate care plans that address the unique needs of maternal and child patients. Utilize electronic medical record (EMR) systems efficiently to document patient assessments, progress notes, and educational interventions. Requirements Valid nursing license with current registration; experience in maternal-child health preferred. Demonstrated expertise in diabetes management, including knowledge of physiology related to endocrine function. Experience with care planning, diagnostic evaluation, and implementing nutrition or dietary interventions. Proficiency in health coaching techniques aimed at behavior modification and weight management. Familiarity with EMR systems used within hospital environments for documentation purposes. Long-term care experience, an acute care background, or hospital settings are advantageous.
