Job Responsibilities Identify, promote and implement activities and wellness programs at the community dialysis center. Manage screenings, physical assessments, data compilation, analysis, and recommendations for follow-ups for dialysis patients. Conduct regular reviews to monitor patients' compliance and progression in the exercise program and ensure care plans are followed through. Maintain exercise and program equipment and ensures regular disinfection and servicing. Manage patients' attendance to ensure accurate and timely documentation to maintain consistent participation. Collaborate with Clinical Services Team to address any barriers to participation and ensures patients receive the necessary support to continue their wellness journey. Assist in organizing volunteer events and outings to empower seniors and encourage engagement. Any other matters as required. Job Requirements Minimum Diploma in Health Sciences (Health Management) or other relevant courses (e.g., sports sciences, therapy aide, community care associate). Relevant working experience in a healthcare setting is advantageous. Strong interpersonal skills and proficient in Microsoft Office.
