We are seeking a Patient Care Coordinator•LVN/LPN or RN.Must have a current nursing license.
You will coordinate a care and coaching program with remotely monitored patients.
This program focuses on diet and exercise. An increased knowledge based of macronutrients, calories, strength training, cardio, etc. is preferred.
This is a 100% work from home position. Anywhere in the USA.Some latitude to create your own schedule.
Engage new patients enrolled.
Help patients track and report changes in condition.
Update medical record systems with condition changes.
Assist patients with healthcare questions and concerns.
Help patients achieve goals.
Experience in health coaching and weight management is an asset.
Experience in a patient-facing role.
Previous medical clinic and work from home experience is an asset.
Current Nursing license.
Strong interpersonal qualities.
Strong organizational and time management ability.
Strong technical skills. Excellent written and verbal communication skills.
