Job Description
As a Transition of Care Coach (RN), you will play a vital role in ensuring seamless transitions for members from hospital admission to various care settings. Your focus will be on reducing readmissions and coordinating comprehensive support for patients, collaborating with hospital discharge planners and other healthcare professionals. This is a hybrid position requiring approximately 20% travel to hospital settings. Additionally, 40-50% local travel may be required (based upon state/contractual requirements).
