Linkage Coordinator job description:
Seeking community‐oriented professionals to be part of an exciting demonstration project to screen and enroll adults with intellectual and developmental disabilities (IDD), TBI, and mobility impairments, who are currently lacking recommended preventive health care and coordinate linkages to accessible preventive healthcare services and health promotion programs.
Essential job functions:
Complete the linkage coordinator training provided by the Utah Department of Health, Disability and Health Program. Provide information in a manner that is inclusive and appropriate to the needs of the population being served and ensure accessibility and usability tools and resources for individuals with disabilities in accordance with the Americans with Disabilities Act and Section 504 of the Rehabilitation Act. Provide information and services in a fair, accurate and impartial manner. Screen and enroll adults with IDD who are currently lacking recommended preventive healthcare. Conduct initial and follow‐up sessions with participants to: Assess participant knowledge of recommended preventive health care and healthy behaviors Provide education about the importance of scheduling health check‐ups with providers for primary care, well‐woman, mental health, dental, vision, and hearing needs Screen participant for depression and anxiety Identify barriers to engaging with recommended preventive healthcare and health promotion programs Provide referrals and facilitate linkages to preventive health care and referrals andfacilitate linkage to preventive health promotion programs in the community and to essential services that support engagement with health services (e.g., financial assistance, healthcare benefits, caregiver support, food, housing, transportation, substance use treatment services, employment/vocational training, education, and other social services). Monitor and record participants' progress, barriers/challenges and successes while enrolled. Conduct a 3‐month, post‐referral, follow‐up session with participants to: Assess increases in participant knowledge about recommended preventive health care and healthy behaviors Determine whether participants received recommended services or adopted healthy behaviors (e.g., physical activity, healthy eating, smoking cessation, etc.) Identify barriers to receiving recommended services and adopting healthy behaviors Address barriers (e.g., further facilitate linkages to appropriate services and resources) Proficient using technology to conduct outreach, enrollment, and other work duties. Collect and report data to the Utah Department of Health, Disability and Health Program. Support development of new Take Care Utah services including Housing Related Services and Supports. Conduct individualized case management to identify the unique needs of vulnerable individuals and families within our community beyond Linkage Coordination Actively link clients to critical external resources, including housing assistance, medical care, food banks, and mental health services. Abide by all state, health district and agency policies regarding confidentiality of patient Information. Perform other duties as assigned.
Qualifications:
Bachelor's degree in related field; or equivalent work experience Knowledge of federal and state health care programs; social determinants of health; case management processes Speak, listen and write in a clear, thorough and timely manner Demonstrate a willingness to be flexible in changing environment while maintaining effectiveness and attention to detail Positive mindset and ability to maintain strong working relationships with co-workers, consumers, and other community and government partners Comfort level and desire to work independently to initiate and carry out job duties.
