Our Wellness Coaches are responsible for member outreach for those in need of increased support and services as the result of foster care transition, high-risk pregnancy, or mental health conditions. This position will facilitate care coordination with medical and mental health providers and community resources as well as engage with members to ensure goals are achieved, with an emphasis on care for youth engaged in the foster system and pregnant mothers diagnosed with high-risk pregnancies and their newborns. REQUIRED
EDUCATION and
EXPERIENCE
Bachelor's degree in social work, psychology, nursing, counseling, or related behavioral health field Experience working with individuals engaged in the foster system or with mother's experiencing high-risk pregnancies (preferred) A minimum of 6 months of outreach/case management experience (preferred) QMHP registration (preferred) Valid Virginia Driver's license, proof of current insurance and willingness to use personal vehicle. Driver's license is an essential requirement for performing duties for this position Clean Motor Vehicle Record - no more than 2 moving violations or a license suspension in past 3 years This position must be agreeable to attend training for position in Richmond, VA
ESSENTIAL DUTIES/RESPONSIBILITIES
Coordinate intensive outreach and case management services to members to avoid gaps in services and to provide continuity of care. Assists members in implementation of Crisis and Safety planning. Communicate openly and effectively with treatment team. Coordinates with medical and mental health providers and additional resources to enhance the members network of care. Successfully coordinate with providers to ensure member is receiving proper services and treatment, to include transport and advocacy during appointments. Collects data to measure outcomes. Report progress throughout duration of services. Discuss issues with Supervisor under direction of Director of Community Based Services and coordinate interventions Advocates for members to ensure delivery of services. Follow-up with member to facilitate appropriate linkages until client is stable and ready for transition to standard case management or other service options. Maintain all appropriate records related to job duties, including comprehensive assessments and service plans. Work independently and flexibly to ensure schedules and service needs of all clients are met. About the
