Job Description

JOB SUMMARY
Provides individualized diabetes self-management education/training to individuals and groups according to Scope of practice, Standards of Practice, and Standards of Professional Performance for Diabetes Educators. A Diabetes Education Program will have at minimum an RD and an RN as instructors.
PRINCIPLE FUNCTIONS
include the following, other duties may be assigned: Maintain or obtain 15 hours of CEU yearly, specific to diabetes, approved by recognized providers. Assesses the individual with diabetes in a collaborative and ongoing manner to provide individualized learning plans. Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with DSME and support program participants. Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods. Collaboratively develops an individualized follow-up plan with each program participant. Evaluates the effectiveness of educational services provided by measuring learning objectives. Conducts follow-up assessment upon completion of the DSME program services, using follow-up form evaluating behavioral, long-term outcomes, glycemic and patient goals which is based on the AADE7 framework. Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records. Utilizes a team approach to provide services and collaborates and communicates with team members when needed. Identifies when a program participant's needs are outside the scope of the instructor's practice and expertise, plus arranges for additional services to meet needs. Assists with navigation through healthcare systems. Provides awareness and education about diabetes to community. Acts as a resource for providers and other health care members. Performs consults and appropriate follow up as requested by WhidbeyHealth providers which may include inpatient, cancer infusion center, wound care, emergency department, Obstetrics, and WhidbeyHealth clinics. Assist with collection of patient statistics data and quality improvement projects for annual report and re-certification every 4 years.
JOB KNOWLEDGE & QUALIFICATIONS
Education Required:
Graduate of an accredited school of nursing
Preferred:
BSN/MSN or working towards degree. Training and Experience Minimum of 6 months' experience providing Diabetes Self-Management Education (DSME) and Support (DSMS). Certificates, Licenses, Registrations Active WA State RN License or Active Multistate License (MSL)
MSL Requirements:
Approved 6-Hour Suicide Prevention Training Course Completion Screenshot of submitted demographic data using the Washington State Multistate Nurse Demographic Data Survey. American Heart Association BLS Required. Certified Diabetes Care and Education Specialist (CDCES) or Board Certified in Advanced Diabetes Management (BC-ADM) preferred. Other Skills and Abilities In-depth knowledge about current diabetes treatment management. Knowledge of standards of diabetes education. Able to lead and effectively manage groups. Ability to develop a collaborative, therapeutic alliance with individuals. Basic computer skills.
