In this vital role, you will provide high-quality, person-centered care to individuals in a Residential Care Facility (RCF) setting. Your expertise will support residents and clients with daily activities, health monitoring, and emotional well-being, ensuring they receive respectful and comprehensive care. Duties Assist residents and clients with activities of daily living such as bathing, dressing, grooming, mobility, and toileting Administer medications accurately following medical protocols and document all medication administration in electronic health record (EHR) systems like Epic or Cerner Monitor vital signs including blood pressure, pulse, temperature, and respiration to track health . Maintain detailed medical documentation adhering to HIPAA regulations and ensure confidentiality of patient records Collaborate with nursing staff and healthcare teams to coordinate patient care plans and report any changes in condition promptly Provide emotional support and companionship to enhance residents' quality of life in a mental health setting. Experience Prior experience working in assisted living facilities, nursing homes, hospitals, or home care settings is highly preferred Knowledge of medical terminology, physiology, anatomy, and medication administration Familiarity with EMR/EHR systems like PCC, Point Click Care for accurate medical documentation Experience caring for individuals with developmental disabilities or mental health disorders. Demonstrated ability to work with diverse populations including seniors and individuals with disabilities Strong understanding of HIPAA compliance and medical records management Certification as a CMT, LIMA or relevant caregiving credentials is advantageous; willingness to obtain necessary certifications is required Join us to be part of a dedicated team committed to delivering exceptional patient-centered care.
