https://ask-health.net/ Position Overview We are seeking a compassionate and dependable Patient Care Technician (PCT) to support our Advanced Practice Providers (NPs and PAs) who deliver care to patients in Skilled Nursing Facilities (SNFs) through telemedicine. This role serves as the on-site clinical extension of the provider, assisting with patient assessments and facilitating virtual visits. The PCT will travel to local SNFs, work directly with patients during telehealth encounters, and help ensure providers have the clinical information needed to deliver high-quality care. This is an excellent opportunity for someone who enjoys patient interaction, values teamwork, and is eager to learn new clinical skills in a dynamic healthcare environment. All onsite training will be provided. Key Responsibilities Telemedicine Visit Support Serve as the on-site clinical assistant during telemedicine visits between patients and providers. Prepare patients for virtual appointments, including gathering vital signs and clinical information. Assist providers with patient assessments by performing directed physical exam components as instructed. Operate telehealth equipment and ensure smooth virtual connectivity during visits. Document basic visit information and relay patient updates to the provider as needed. Clinical Duties Obtain and record vital signs and other clinical measurements. Assist with neurological screening procedures as directed. Perform Creyos Cognitive Assessments and Diabetic Peripheral Neuropathy (DPN) screenings following training and established protocols. Support providers with patient education and follow-up instructions. Maintain accurate documentation in the electronic medical record (EMR) system when required. Facility Coordination Travel to assigned Skilled Nursing Facilities (SNFs) to support scheduled patient visits. Coordinate with SNF nursing staff to identify patients scheduled for telemedicine visits. Ensure patients are ready and available for scheduled provider consultations. Maintain a professional and collaborative working relationship with facility staff. Operational Support Ensure telemedicine equipment is functional and ready for use. Communicate patient concerns or clinical observations to the provider promptly. Follow all infection control, HIPAA, and facility protocols. Maintain patient dignity, privacy, and comfort at all times. Qualifications Required High school diploma or equivalent. Ability to travel regularly to local Skilled Nursing Facilities. Strong interpersonal and communication skills. Comfort working in a variety of healthcare environments. Ability to work independently while supporting a remote provider. Preferred Previous experience as a Patient Care Technician, Medical Assistant, CNA, or similar clinical support role. Background in medical terminology. Experience working in Skilled Nursing Facilities or long-term care settings. Familiarity with telehealth or virtual care environments.
Pay:
$23.00 - $27.00 per hour
