Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to promote independence and dignity. Monitor and observe patients' conditions, including vital signs, medication adherence, and overall health status. Support patients with mobility and transfers using patient transfer methods and equipment like Hoyer lifts or wheelchairs. Administer medications as prescribed, ensuring proper documentation in medical records or electronic health record (EHR) systems. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory-related conditions, including memory care techniques. Assist with medical procedures such as catheter care and wound management under supervision. Maintain patient confidentiality in compliance with HIPAA regulations while documenting medical records accurately. Support end-of-life care with compassion and professionalism, respecting patient wishes and comfort measures. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment. Communicate effectively with clients' families and healthcare teams to coordinate care plans and update on patient progress. Experience Candidates should have prior experience working in home healthcare settings or assisted living facilities. Experience working with individuals with developmental disabilities or geriatrics is highly valued. Familiarity with EMR (Electronic Medical Records) or EHR systems is preferred for efficient documentation. Candidates must demonstrate knowledge of medical terminology, home health care regulations, and patient safety protocols such as first aid and basic life support (BLS). Previous roles involving hospice or heavy lifting tasks are advantageous. Ideal applicants will have hands-on experience assisting with ADLs, medication administration, vital signs monitoring, and patient transfer methods like Hoyer lifts.
