Job Description

40 hours per week (Mon - Fri 6am-2pm) The pay range is $22.75-24.30 per hour based on experience You must have a valid NY State driver's license and a HS diploma/GED to be considered for this position. The individuals at this location need some ADL supports as they have moderate to low capabilities. Lifting adult individuals is necessary using a 2 person lift and equipment.
Qualifications include:
High School Diploma or equivalent Prior experience working with persons with developmental disabilities preferred Minimum six months' experience with Life's WORC or similar agency in capacity of Support Counselor or in another health related field Excellent verbal and written communication skills Valid NY State Driver's License required
Responsibilities Include:
Accompany and advocate for individuals on medical/dental appointments and emergency room visits Schedule all medical appointment in coordination with the Nurse and Manager Maintenance of a medical appointment calendar in coordination with the Nurse and Manager Meets with the Nurse and Manger weekly to coordinate information brought to and from appointments Provide ongoing protective oversight to individuals living in the residence Implementation of active treatment Provide assistance and instruction to each individuals in basic activities of daily living Provide training and support for each individuals in accordance with the individual program plan Perform daily check of medication and supplies Successful completion of the AMAP course and annual recertification for administration purposes Scheduling flexibility as needed; may be required to work beyond the normal shift Drive agency vans that are equipped with GPS monitoring systems and demonstrate safe driving practices Assist in maintaining a clean and safe environment; proper use and maintenance of household equipment and appliances
