Job Description

As a Hospice Clinical Supervisor, you will report directly to the Clinical Director and serve as a key member of the operations and management team by leading a full team of multidiscipline, healthcare professionals. Your primary work will include the supervision, management, coordination and provision of quality patient care while demonstrating fiscal responsibility and maintaining the highest standards of care. You may occasionally provide one on one hospice care to patients in the home setting by conducting supervisory visits, new admissions or on call.
  • Participates with the physician, referral sources, and professional staff in decisions regarding patient eligibility and suitability for the hospice program and coordinates hospice care services from admission through bereavement.
  • Coordinates with physicians, case managers, the patient, families and interdisciplinary team to implement the patient plan of care.
  • Coordinates regular interdisciplinary team conferences and promotes communication of information relevant to the patient care process to ensure quality of care and coordination of services.
  • Participates in agency quality assurance activities designed to improve quality and continuity of patient care which includes reviews of medical records, ensure adequate staffing, training and evaluating staff.
  • Other job duties as assigned by Clinical Director.
JOB QUALIFICATIONS
Current RN License :
A valid nursing license is essential for this role.
Clinical Experience :
Several years of clinical experience, preferably in a supervisory or managerial role.
Hospice experience:
at least 1 year of previous hospice experience preferred
Education :
A bachelor's or master's degree in nursing or healthcare administration is highly desirable.
Leadership Skills :
Strong clinical knowledge, excellent communication skills, and proven leadership experience.
Regulatory Compliance :
Familiarity with electronic health record systems (HCHB) and compliance with healthcare regulations.
Team Management :
Ability to manage schedules, provide guidance, and mentor nursing staff. 1 year of previous management experience preferred.
Patient Care Focus :
Commitment to delivering high-quality patient care and supporting the professional development of nursing staff. These requirements ensure that the RN Clinical Supervisor can effectively oversee nursing staff, coordinate patient care, and maintain high standards of patient care within the hospice setting.
