Job Description
Job Overview The Patient Care Coordinator will have a strong interest in improving health outcomes for patients, with the ability to easily communicate with diverse patients and medical staff. The Care Coordinator will be a self-starter, driven by improving health outcomes and health equity with the ability to operate and perform duties with minimal support. What You'll Do Provide exceptional friendliness, empathy and customer service via phone and in-app messaging Assist patients in scheduling stat and urgent orders Effectively and accurately respond to inquiries from patients, their relatives and or referring physicians and provide accurate information in a timely manner Owning the response and actions related to handling faxes, live chats, emails and secure messages via the c-lynx App Developing and suggesting process improvements that will have a direct impact on the benefit patients, clinicians, colleagues and overall operations About You Strong belief that patients should have greater access to their healthcare team, to their own health information, and a greater ability to make informed decisions about their care A caring individual with true compassion who is excited to build a new model for healthcare delivery 1+ years of care coordination or navigation experience Creative, flexible, and always puts the patient first An ability to remain focused and calm during challenging situations Excellent written, verbal and visual communication skills with meticulous attention to detail Ability to stay organized and balance several projects simultaneously Prior healthcare or clinical experience This role is essential for fostering positive patient experiences and supporting the overall operations of the healthcare facility.
