Located in beautiful Northern California, Adventist Health Clear Lake has been a leading healthcare provider in Lake County since 1968. We are comprised of a 25-bed critical access hospital and provide primary and specialty care services at clinics throughout the area to best serve our community. Lake County is home to Clear Lake, California's largest natural freshwater lake, surrounded by charming small towns, beautiful mountains, vineyards and orchards. This area is perfect for outdoor enthusiasts, with some of the best bass fishing, rural trails for hiking and biking, all the while being only two hours from the San Francisco Bay Area, Sacramento Valley or Pacific Coast.
Job Summary:
Supports outpatient clinic physician and interdisciplinary teams in facilitating patient care with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction. Maintains responsibility for implementing day-to-day care management interventions for identified clinic patients. Coordinates small program(s) with limited budget/impact. This position is represented by RCHEA.
Job Requirements:
Education and Work Experience:
High School Education/GED or equivalent: Preferred Associate's/Technical Degree or equivalent combination of education/related experience: Preferred Five years' technical experience:
Preferred Licenses/Certifications:
Medical Assistant (MA) certification:
Preferred Essential Functions:
Coordinates and monitors patient flow and staff activities to ensure prompt, courteous and accurate response to patients. Ensures effective clinic organization through planning, leading, controlling and organizing clinic activities. Works collaboratively and maintains active communication with physicians, nursing and other members of the multi-disciplinary care team to affect timely, appropriate patient management. Utilizes advanced conflict resolution skills as necessary to ensure timely resolution of issues. Conducts telephonic initial and follow-up assessment of clinic patients. Participates in program evaluation and quality improvement activities. Performs other job-related duties as assigned.
