  • Assisted Living Assisted Living & Memory Care community in Cerritos seeks an energetic, dedicated Nursing professional to serve as Health Services Director.
This individual will direct all medication and healthcare-related services within the facility. Primary responsibilities include ensuring the following: Initial and ongoing assessment of resident needs. The timely development of initial individual care plans and completion of required updates. Regular monitoring and documentation of resident health and scheduling of medical appointments. The timely reporting of resident health and changes in condition to regulatory agencies. Ensure that all resident health records and documentation adhere to regulatory standards. Monitoring of outside healthcare and hospice services to ensure adherence to regulatory requirements. The consistent delivery of plan specified care and medication management services. Oversee medication management systems and practices ensuring safe administration, complete and accurate documentation, appropriate packaging and storage. Maintain inventory of healthcare supplies and equipment and follow Company protocol for purchasing. Timely scheduling, supervision and training of Caregiving and Medication staff. Regular communication with outside medical professionals, pharmacies, service providers and families. We seek a mature and caring individual with superior service, communication and leadership abilities. A current State of California LVN license and at least five (4) years of direct care experience are required. Memory Care experience and RCFE Certificate a plus.
