El Camino Health Medical Network is currently seeking a talented Referral Coordinator to join our growing healthcare team!

8:00am-5:00pm El Camino Health Medical Network is recruiting a full-time Referral Coordinator for our El Camino Health Primary Care and Multi-Specialty Care outpatient practices located in Santa Clara County. The Referral Coordinator serves as an educator, healthcare resource, and patient advocate to ensure that patient care is effectively coordinated and managed. This role is responsible for referring patients to external physicians and specialists, acting as a liaison between providers, and ensuring seamless care transitions across the healthcare continuum.Essential Functions:
Patient Coordination & Communication:
- Greets patients and visitors in person or over the phone in a courteous and professional manner.
- Responds to inquiries from patients, family members, physicians, and staff with knowledge and professionalism.
- Accurately documents and communicates messages to appropriate personnel in a timely manner, following department procedures.
- Facilitates effective communication between physicians, clinical managers, medical assistants, consultants, and team members.
- Ensures timely and accurate documentation of referral information in the electronic medical record (EMR).
- Addresses and escalates any issues related to scanning, indexing, and EMR functionality to IT or EPIC support.
Referral Processing & Care Coordination:
- Coordinates referrals to external physicians and specialists, ensuring compliance with payer and regulatory requirements.
- Acts as a liaison between patients, providers, and external healthcare organizations to ensure timely and efficient referral completion.
- Tracks and follows up on referrals to ensure all necessary authorizations, appointments, and documentation are completed.
- Supports physicians and medical staff in facilitating appropriate patient care and treatment plans.
Operational Efficiency & Compliance:
- Contributes to a positive patient experience and high satisfaction ratings.
- Participates in performance improvement initiatives to enhance workflow and service delivery.
- Adheres to all HR, safety, HIPAA, and compliance policies.
- Maintains attendance and punctuality in accordance with department standards.
- Performs assigned duties in alignment with established schedules, policies, and procedures.
- Completes other tasks and responsibilities as assigned to support department operations.
Minimum Requirements:
- High School Diploma or GED required.
- Minimum of five (5) years of experience in a healthcare setting (acute care or ambulatory care), with a preference for experience in utilization management, case management, or care coordination.
- Experience working with an electronic medical record (EMR) system, preferably EPIC.
- Ability to type a minimum of 40 words per minute accurately.
- Strong verbal and written communication skills with the ability to interact professionally and courteously.
- Excellent active listening skills and ability to engage effectively with patients and healthcare professionals.
- Exceptional attention to detail and accuracy in documentation and referral tracking.
- Strong organizational skills with the ability to prioritize tasks and manage multiple responsibilities efficiently.
- Ability to work effectively as part of a team, demonstrating flexibility, cooperation, and a willingness to assist others.
- Solid patient and guest relations skills, ensuring a compassionate and professional approach to care.
Preferred Qualifications:
- Experience with
EPIC EMR
system.- Current California Licensed Vocational Nurse (LVN) license.
