The Circa 60 program provides full service outreach services in Alameda County combining community based housing/financial support, intensive mental health, case management, substance use, primary care, and psychiatric support to older adults (60+) who are chronically homeless and have severe mental health issues with average service duration of three years. This role requires someone who has experience holding staff accountable to high standrds of service delivery.
