You will provide leadership, operational oversight, strategic planning, staff development, and clinical guidance to rehabilitation professionals while partnering closely with interdisciplinary leaders to support participant-centered care. This role combines clinical expertise, leadership, mentorship, and program development into a rewarding position where your decisions directly impact the lives of seniors and the success of your team. What You'll Be Doing Lead and mentor rehabilitation professionals including PTs, OTs, and support staff Oversee rehabilitation program operations, quality initiatives, and clinical outcomes Partner with interdisciplinary teams to coordinate participant care plans Manage staffing, scheduling, program development, and departmental performance Ensure compliance with state, federal, PACE, and rehabilitation regulations Support therapists and participants through complex care needs Conduct home visits when needed to evaluate services and participant needs Drive continuous improvement and excellence in rehabilitation delivery Collaborate with leadership on strategic planning and departmental goals What We're Looking ForRequired California licensed Physical Therapist (PT) or Occupational Therapist (OT) Bachelor's Degree in Physical Therapy or Occupational Therapy Minimum 5 years of rehabilitation experience Minimum 2 years of leadership or supervisory experience Experience treating older adults with complex medical needs Strong communication, mentoring, and team-building skills Preferred Experience working with geriatric populations PACE, home health, SNF, outpatient rehab, or community-based care experience Passion for interdisciplinary, participant-centered healthcare Proven ability to develop and retain high-performing clinical teams Ideal Candidate You're a successful therapist who enjoys helping others grow.
