Job Description

California Dermatology Institute is seeking a highly organized and dependable Patient Care Coordinator to support our medication operations team. This role will focus on coordinating internal communications, nursing facility medication requests, and follow-up between departments to ensure medication needs are addressed efficiently and accurately. The ideal candidate is an excellent communicator who can manage multiple requests, prioritize tasks, and make sure messages are directed to the appropriate team members. Responsibilities Oversee and organize internal medication-related messages and requests. Monitor communication channels and ensure messages are routed to the appropriate department or team member. Help filter and prioritize requests to reduce unnecessary communication to providers and clinical staff. Coordinate medication requests and follow-up for nursing facilities and SNF patients. Communicate with nursing facilities regarding medication orders, refills, status updates, and outstanding information. Follow up on medication requests to ensure they are completed within appropriate timeframes. Coordinate with pharmacy, medication, nursing, clinical, and administrative teams. Identify issues or delays and escalate them to the appropriate lead or manager. Maintain accurate documentation of communications and medication-related requests. Ensure nursing facility requests are communicated clearly and efficiently. Assist with resolving communication gaps between departments. Follow established workflows, SOPs, HIPAA requirements, and company policies. Help ensure providers receive only the messages and requests that require their attention. Perform other coordination and administrative duties as assigned. Qualifications Previous experience in a healthcare, medical office, pharmacy, or patient coordination environment preferred. Strong written and verbal communication skills. Excellent organizational and follow-up skills. Ability to manage a high volume of messages and requests. Strong attention to detail. Ability to prioritize urgent versus routine requests. Comfortable communicating with nursing facilities, healthcare staff, and internal departments. Ability to work independently while collaborating with multiple teams. Experience with electronic medical records or healthcare communication systems preferred. Knowledge of medications or pharmacy workflows is a plus. Pay:
$22.00 - $24.00 per hour
