We are currently seeking a full-time Patient Care Coordinator. As a patient care coordinator, you will play an important role in ensuring that the patient's experience is seamless from the initial point of contact with us to their graduation.

Duties include:

• Answering phones and checking-in patients.
• Help patients feel welcome upon arriving and when leaving their appointments.
• Know the patient's plan of care and ensure they are scheduled as needed.
• Collecting out-of-pocket costs (Co-pays etc.).
• Create and maintain patient records.
• Obtain physician referrals for patients when required.
• Working closely with the clinic director to ensure the schedule remains full so that we can help as many people in the community as possible.
• Helping therapists with patients when needed.
