Full-time:
Sunday - Thursday 10a-6p We are looking for a Clinical Coordinator to join our team! As a Clinical Coordinator, you will partner with the Resident Services Director (RSD) and Resident Care Director (RCD) for all administrative tasks for the Wellness Department.Duties/Responsibilities:
Checks ADL books/resident electronic health record (myUnity) for documentation and completion of care. Checks paper/electronic Medication Administration Records (eMARs) and Medication Observation Records (eMORs) and verifies that medications have been given and treatments completed. Reports any omission to the RCD for follow-up. Develops Resident Care Associate (RCA) staffing schedule in conjunction with the RSD. Responsible for supporting and assisting RCA staff with training and in-service completion Assembles binder/non-EHR Resident Records for new move-ins. Updates resident face sheet information. Enters details of resident care needs into the Yardi/Resident Summary Template (myUnity). Ensures accuracy of RCA Assignments on paper or in Yardi and provides them to staff (if using Yardi/paper system). Assists the RCD with follow-up in obtaining Care Plan signatures. Orders and maintains sufficient supplies for the Resident Care department. Coordinates Quality Assurance and family meetings in conjunction with the RSD. Provides the Receptionist/Concierge with documentation (face sheets, DNRs, medication lists, etc.) for the Resident Emergency binders upon move-in, as required, and when changes occur. Assists with providing information and completing documentation for town/state-funded programs (if applicable).Requirements:
